CASE MANAGEMENT (CMGT)
Case Management consists of actions which help service recipients of all ages and disabilities to gain access to needed medical, social, psychological, vocational, financial, educational, legal, and/or other community resources and services, so they may maintain a better quality of life and live with greater independence and functioning within their communities. It involves the coordination of services to assure continuity of care and accountability for service provision. The general functions of case management include assessment, plan development, referral, coordination and monitoring of recommended services and advocating on behalf of the service recipient.
