Home Health Nursing

Skilled nursing (SN) services are provided on an intermittent basis and are covered by most payors, including Medicare. These services are provided by a licensed Registered Nurse (RN), Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). LPNs and LVNs are supervised by a Registered Nurse.

Home Health Aide (Aide)

Aide services are provided by a qualified Home Health Aide on an intermittent basis to provide personal care. These services are covered by most payors, including Medicare, when other skilled services are also being provided. Aides are supervised by RN, PT, OT or ST.  

Physical Therapy (PT)

PT services are provided by a licensed Physical Therapist or Physical Therapy Assistant (PTA) on an intermittent basis and are covered by most payors, including Medicare. PTAs are supervised by a PT. These services include, but are not limited to gait training, strength training, and provision of a home exercise program.

Occupational Therapy (OT)

OT services are provided by a licensed Occupational Therapist or Certified Occupational Therapy Assistant (COTA) on an intermittent basis and are covered by most payors, including Medicare. COTAs are supervised by an OT. These services include, but are not limited to upper body strength training, improving range of motion skills and provision of a home exercise program.

Speech Therapy (ST)

ST services are provided by a licensed Speech Language Pathologist (SLP) on an intermittent basis and are covered by most payors, including Medicare. These services include, but are not limited to improving swallowing, cognition and speaking.

Medical Social Work (MSW)

MSW services are provided by a Master’s prepared Social Worker on an intermittent basis and are covered by most payors, including Medicare. A Bachelor’s prepared Social Worker is supervised by a Master’s prepared Social Worker. These services include, but are not limited to a psycho-social assessment, helping patient/family access community resources, and counseling.

ACHC Home Health Specialty Service

Psychiatric/BH Home Care (BHHC)

Psychiatric Home Care, also known as Behavioral Health Home Care, integrates behavioral health interventions into home care services for patients whose mental illness, substance abuse, intellectual/developmental disabilities make it difficult, or otherwise prevent them from receiving care outside of their homes. Services are provided by a psychiatric nurse and other home care personnel, as ordered by a physician.
