The Accreditation Commission for Health Care has developed its Transition Program to allow currently accredited organizations the ability to easily switch to ACHC accreditation. The transition process is designed to be as seamless as possible by providing a personally assigned Transition Coordinator and a list of all necessary documentation. Your Transition Coordinator will be available to answer any questions you may have, walk you through each step of the way, and eliminate any hurdles that you may encounter during the transition process. Switching accreditation organizations has never been easier!

	ACHC WILL REVIEW SUBMITTED DOCUMENTS
	TRANSITION COORDINATOR WILL PROVIDE AN AGREEMENT
